How to submit an open records request:

  1. Please know the specific record that you would need.
  2. It is preferred that you request the record in writing, but you can simply state your request. To avoid any dispute or misunderstanding of the record and the time it was requested, you can fill out this this simple form to draft an open records request.
  3. Most records requests can be completed in a timely manner, but don't forget to ask for records in advance. The records custodian has up to three business days to either make the record available or provide a written explanation as to why the specific record is an exception to the Open Records Law.
  4. Also, the law allows for the copying of documents to be up to 25 cents per page. If the retrieval of the record requires significant administrative costs, you could be billed an hourly rate that would not exceed the salary of the lowest paid, full-time employee who, in the discretion of the custodian of the records, has the necessary skill and training to perform the request. 
    • NOTE: It is not  always  necessary to charge for the retrieval of records, and if possible, the records custodian can e-mail the document if you would rather save in printing costs.

ALCOHOL LICENSE INFORMATION

FOR NEW APPLICATIONS:

The state requires that the applicant have their county license before the State will issue its license.  When you receive the County's application package, call the State for an application.  We require a copy of all the paperwork that will be sent to the State before we will accept your application.  You may contact the State at:

You will also need to contact the Putnam County Health Department at 706-485-8591 to obtain your Food Service Permit and the Planning & Development Department at 706-485-2776 to obtain your Occupation Registration.

 

The following must be submitted before we can consider your application:

  1. Completed Application

  2. Accurate sketch or diagram showing location of building and property lines (only on initial applications when premises is not in existence on January 6, 2010)

  3. Detailed set of plans and working drawings (only if new or renovated building)

  4. O.C.G.A. 50-36-1(e)(2) Affidavit

  5. Private Employer Affidavit of Compliance Pursuant to O.C.G.A. 36-60-6(d) OR Private Employer Exemption Affidavit Pursuant to O.C.G.A. 36-60-6(d)

  6. Criminal History Consent Form (turn in directly to the Sheriff's Office)

  7. Copy of all completed state paperwork (see state check lists)

  8. $300 Performance Bond (if Retail Beer/Wine)

  9. Valid Georgia Drivers License or other acceptable Secure & Verifiable Document of owner or agent

  10. Application Fee

The application will not be accepted without all of the above documents.

After we have received all of the above information, we will forward your application to the Sheriff, Fire Marshal, and Building Inspector asking them to perform the necessary background check and building inspections.  No license will be issued until all required information has been submitted and approval has been received from the Sheriff's Office, Fire Marshal, Building Inspector, and the Board of Commissioners.

 

FOR RENEWAL APPLICATIONS:

As specified in the Putnam County Alcoholic Beverage Control Ordinance adopted on March 20, 1990, your alcohol license is valid only for the calendar year indicated thereon and will not be automatically renewed.  A licensee who desires to continue in business during the next year must make a renewal request on or before December 31st, reporting all changes since the last application.

There are two renewal options:

Option A:  If anything on your application from last year has changed, the following must be submitted before we can consider your renewal application:

  1. Completed Application

  2. Detailed set of plans and working drawings (only if new or renovated building)

  3. O.C.G.A. 50-36-1(e)(2) Affidavit

  4. Private Employer Affidavit of Compliance Pursuant to O.C.G.A. 36-60-6(d) OR Private Employer Exemption Affidavit Pursuant to O.C.G.A. 36-60-6(d)

  5. Criminal History Consent Form

  6. $300 Performance Bond (if Retail Beer/Wine)

  7. Copy of Current State License

  8. Valid Georgia Drivers License or other acceptable Secure & Verifiable Document of owner or agent

  9. Application Fee

Option B:  If nothing on your application from last year has changed, the following must be submitted before we can consider your renewal application:

  1. Completed Application

  2. O.C.G.A. 50-36-1(e)(2) Affidavit

  3. Private Employer Affidavit of Compliance Pursuant to O.C.G.A. 36-60-6(d) OR Private Employer Exemption Affidavit Pursuant to O.C.G.A. 36-60-6(d)

  4. Criminal History Consent Form

  5. $300 Performance Bond (if Retail Beer/Wine)

  6. Copy of current State license

  7. Valid Georgia Drivers License or other acceptable Secure & Verifiable Document of owner or agent

  8. Application Fee

The application will not be accepted without all of the above documents from either Option A or B.

The annual fee shall be paid at the time application is made for the license renewal.  No license will be issued until all required information has been submitted and approval has been received from the Sheriff's Office, Fire Marshal, Building Inspector, and the Board of Commissioners.

YOUR RENEWAL APPLICATION MUST BE SUBMITTED NO LATER THAN DECEMBER 31ST OF EACH YEAR.  IT IS YOUR RESPONSIBILITY TO ENSURE THAT YOUR LICENSE IS RENEWED.

LODGING PERMIT INFORMATION

FOR NEW APPLICATIONS:

Before anyone can operate rooms, lodging or accommodations in Putnam County, they must obtain a Lodging Permit.  The tax is not due for an individual stay over 30 days.

Each month, the permit holder is required to mail the monthly report (even if no rent is collected for the month) along with the tax due to Putnam County.

 

The following must be submitted before we can consider your application:

  1. Completed Application

  2. O.C.G.A. 50-36-1(e)(2) Affidavit

  3. Private Employer Affidavit of Compliance Pursuant to O.C.G.A. 36-60-6(d) OR Private Employer Exemption Affidavit Pursuant to O.C.G.A. 36-60-6(d)

  4. Valid Georgia Drivers License or other acceptable Secure & Verifiable Document of owner or agent

  5. Application Fee of $1.00

The application will not be accepted without all of the above documents.

After we have received all of the above information, we will issue the permit.  Assuming there are no problems with the application, the permit should be issued within 5-10 business days.

 

FOR RENEWAL APPLICATIONS:

Your lodging permit is valid only for the calendar year indicated thereon and will not be automatically renewed.  A permitee who desires to continue in business during the next year must make a renewal request on or before December 31st, reporting all changes since the last application.

The following must be submitted before we can consider your renewal:

  1. Completed Application

  2. O.C.G.A. 50-36-1(e)(2) Affidavit

The annual fee of $1.00 shall be paid at the time application is made for the permit renewal.  No permit will be issued until all required information has been submitted.

YOUR RENEWAL APPLICATION MUST BE SUBMITTED NO LATER THAN DECEMBER 31ST OF EACH YEAR.  IT IS YOUR RESPONSIBILITY TO ENSURE THAT YOUR PERMIT IS RENEWED.
